Thinly slice the red onion.

Boil water in a pot, then add 2 tsp salt and spaghetti. Cook according to package instructions.

Heat olive oil in a pan on medium heat until hot but not smoking. Add sliced onions and caramelize for 6-7 minutes over medium heat.

Add chopped garlic, chili oil, and cream to the caramelized onions. Stir well.

Stir in Italian seasoning and salt to taste. Optionally, add grated Parmesan and chopped parsley.
